Keeping My Chest Healthy is a project developed by Bradford District Care NHS Foundation Trust, created with and for people with learning disabilities.
Co-produced with people with learning disabilities, families, carers and professionals across health and care, KMCH is a practical, evidence-informed model designed to improve respiratory outcomes and reduce health inequalities.
Aligned with the three key shifts in the NHS 10 Year Health Plan, KMCH supports earlier identification of modifiable respiratory risk, consistent multidisciplinary working, and prevention-focused care in everyday practice.
The KMCH approach
Keeping My Chest Healthy brings together four connected elements:

An evidence-based, multidisciplinary respiratory pathway that supports consistent practice across services. - Personalised, accessible care planning
Care plans that make sense to the person and to the people supporting them, focusing on everyday management and early recognition of change. - A digital health hub
A resource that supports awareness of respiratory health and highlights the modifiable risk factors that are often missed in routine care. - Support for professionals
CPD-accredited training and a full suite of practical clinical tools that link directly to the pathway and support confident, standardised care.
